Section I: Acceptance of the Agreement Welcome to the Zilis™ business. Please read the Zilis™ Ambassador Terms & Conditions and Zilis™ Policies & Procedures carefully, collectively referred to as the “Ambassador Agreement.” Section II: Autoship Zilis allows its Customers and Ambassadors to select a monthly automatic shipment (autoship) for their desired products. Charges are incurred each month with a credit or debit card maintained on file with Zilis. You may make adjustments or cancel your monthly autoship several ways: within the orders section of your Cloud, by submitting a support ticket, or contacting Support at support@zilis.com. Section III: E-SIGN Notice - Consent to Electronic Record E-SIGN, the Electronic Signatures in Global and National Commerce Act (15 U.S.D. 7001, et seq.), requires that you consent to entering into an electronic agreement with Zilis, LLC (“Zilis”) before a Zilis Ambassador Agreement can be executed. Please read the following information carefully: If you enter into an online Ambassador Agreement with Zilis, you will not be required to submit a paper application. An electronic record will evidence the entire agreement between you and Zilis. However, you must consent to the use of an electronic record and must read the Zilis™ Ambassador Terms & Conditions and Zilis™ Policies & Procedures, and electronically acknowledge below that you have read, understand, and agree to the terms of these documents. To access these documents and submit your online Ambassador Application, you will need a personal computer or Mac with Internet access, operational Internet browser software, and Adobe Acrobat Reader. Should there ever be a change in the equipment or software necessary to access the terms of the Ambassador Agreement, Zilis will advise you of the same and will provide you with a list of the equipment and software that is necessary. Upon such event, you may withdraw your consent. Your consent will apply to all transactions between you and Zilis. Section IV: IRS W9 Information The Tax ID number must match the name as shown on your income tax return to avoid backup withholding. For individuals, this is your social security number. Please verify the information submitted on your enrollment form. If correct, acknowledge by checking the box below, which will serve as your digital signature.
